Position Highlights:

  • Position: Registered Nurse Case Manager
  • Hourly Pay Range: $35.55-59.15. The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.
  • Location: Elmhurst Hospital
  • Full Time: 40 hours a week
  • Hours: Monday-Friday, 8am-4:30pm with every 5th weekend and holiday requirements

What you will do:

  • Demonstrates the knowledge and skills necessary to provide care appropriate to the age of patients served. This includes knowledge of the physical and psychological needs of patients served and the ability to respond appropriately to those needs.
  • Performs case management duties toward targeted patient care, and appropriate resource utilization goals and targets. Guide the team to provide safe and effective care progression.
  • Identifies patient and family needs as it relates to continuity and efficiency of care.
  • Works in collaboration with the physician to assess patient needs, develop a plan of care and facilitate implementation of the care plan.
  • In conjunction with other members of the health care team, coordinates care for patients with complex needs and facilitates discharge planning. Works closely with Utilization Review Coordinators to provide necessary information to payers to justify admission and continued stay.
  • Serves as a clinical expert/resource to staff with respect to resource utilization and patient care. Problem solves with staff to aid in the development of critical thinking skills.
  • Participates in the design and implementation of pathways and other clinical improvement activities.
  • Mentors staff and provides informal staff education and role modeling.
  • Enhances patient satisfaction facilitating interaction and communication among members of the health care team and problem solving with staff and physicians. Queries patients and families to identify unmet needs and opportunities for improvement.
  • Maintains clinical expertise through participation in their professional organization, ongoing education and up to date with current literature.

What you will need:

Required Education and/or Experience:

  • Bachelor's Degree in Nursing or Bachelor's Degree in health care related field with case management experience
  • A minimum of one year of previous case management and/or clinical specialist experience

Required License and/or Certification:

  • Current State of Illinois Registered Nurse licensure
  • Current CPR certification issued by either American Heart Association or Red Cross

Preferred License and/or Certification:

  • Certification in case management

Summary:

Provides case management for patients in the acute care setting. In collaboration with the physician and other health care team members, the RN Case Manager coordinates the patient's care to achieve optimal patient outcomes.
