SUMMARY

The Manager of the Remote Patient Monitoring program will lead in the centralized remote patient monitoring program for Grady. This program will support inpatient to home transitions as well as ambulatory care coordination programs, including receiving referrals from providers, confirming patient goals, initiating the patients on the equipment and ongoing monitoring. The Manager will interface with the Mobile Integrated Health team for continuous monitoring and response to alerts. The Manager will also collaborate with IT for equipment selection, evaluation, implementation and upgrades. The Manager will participate in the Patient Care at Home team.

QUALIFICATIONS

• Master's Degree in Nursing (MSN)

  Georgia Nurse Practitioner Advanced Practice

  
  
• 
  

  Georgia RN License

  
  
• 
  

  Basic Life Support (BLS) certification

  
  
• 
  

  Current Georgia License as a Registered Professional Nurse

  
  
• 
  

  Current Georgia License as a Clinical Nurse Specialist (CNS) or Advanced Practice Nurse

  
  
• 
  

  Leadership experience preferred
