We have an exciting opportunity for an experienced Registered Nurse (RN) Care Manager to join our team at MaineHealth Pediatric Gastroenterology!The RN Care Manager will work with pediatric patients whose chronic GI conditions are poorly managed and impact their quality of life, symptom management, and utilization of services.As a trusted healthcare team member, the RN Care Manager works with the patient and family to identify goals that improve quality of life, patient experience, and clinical outcomes and reduce avoidable ED/Hospital utilizations. The RN Care Manager will review the patient's current status, provide ongoing education regarding condition self-management, identify Social Driver of Health (SDOH) related barriers, place community resource referrals, and bridge the coordination of services.This position is full-time and hybrid, with two remote days and three office days after orientation.The position is physically located at 887 Congress St, Portland. Pediatric or GI experience is not required but preferred. Summary: The Registered Nurse - Care Manager role is responsible for managing high-risk patients to promote effective education, self-management support, and timely healthcare delivery to achieve optimal quality and financial outcomes, as well as ensuring safe and effective transfers in the movement of patients across the health care continuum, serving as the bridge between the professional staff in a care setting (e.g. hospital) and the patient and/or family.
