The department is seeking an experienced Case Manager to join their leadership team. At the direction of the department manager, the Care Management Supervisor shares responsibility for day to day operation of the care management department including direct supervision of the staff. Facilitates patient centered care, interactive management and collaboration within the care management department, interprofessional team and the community that serves our patient population. The Supervisor has primary responsibility facilitating patient flow. They demonstrate expertise and the ability to implement timely transitions of care with plans that are safe, effective and focused on preventing unplanned returns to the ED and or hospital. This role models the mission, vision, and core values of the organization; supports evidenced based practice to insure optimal outcomes. Pay Transparency Act Annual Full Pay Range: $128,800 - $252,200 (will be prorated if the appointment percentage is less than 100%) Hourly Equivalent: $61.69 - $120.79 Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable). |
