Nurse Practitioner Physician Assistant Detroit (DNW) Same Day Full Time Day Shift

Position is located in Northwest Detroit at our clinic on Outer Drive. Schedule is Days, Monday - Friday. 1 weekend morning a month is required.

• Advanced Practice Professional will provide health care and specialized health services to patients and families enrolled in Primary Care Medicine.
• The APP will coordinate care of this patient population in collaboration with all members of the health care team.
• This position will be 100% outpatient.
• APP will provide outpatient medical care by taking patient history, performing physical examinations, orders, and interpreting tests, making diagnoses, and institutes appropriate treatment programs.
• Independently evaluates patients in the clinic.
• Ordering/interpreting testing and lab results, modifying plan of care to address results
• Provide clinical expertise and direction to support patient medication management
• Nurse Practitioner or Physician Assistant will maintain accurate documentation regarding patient status, evaluations, and treatments to physicians
• Streamline the patient transfer process between levels of care, including communicating specific patient needs to internal and external team members and other ambulatory care providers
• NP or PA will perform other related duties as required to assure patient safety and satisfaction
• APP will participate in the development and presentation of educational experiences for patients, families, and health care professionals

GENERAL SUMMARY: Provides inpatient and/or outpatient medical care. Takes patient history, performs physical examination, orders, and interprets tests, makes diagnoses, and institutes appropriate treatment programs. Works in collaboration with staff physicians, house officers, nurses, and other medical care team members to ensure that patient outcomes are achieved safely and efficiently.
