Health Care Associate (Philadelphia, PA): Provides individualized support to those with health disabilities in an apartment setting by way of basic and interpersonal skill development and daily supervision while always displaying respect and courtesy for others; acts as an advocate; promotes recovery by supporting the individual in developing relationships and environments that provide hope, empowerment, choice, and opportunities. Requires a high school diploma (Associate degree preferred), experience working with individuals with disabilities, current driver's license. Community Inclusion Specialist (Media, PA): Transport residents using Elwyn vehicles to and from community and recreational activities. Coordinate activity schedule/calendar with other CI team members to ensure the residents have opportunities for the residents to engage in volunteer and other opportunities experience in programs supporting people in behavioral health direct care and developing behavioral support plans. Previous supervisory experience preferred. Strong background in behavior modification, preferred. Program Supervisor: Ensure resident treatment plans are followed. Ensure resident treatment plans are followed. Provide clinical support for client community integration. Bachelor's degree in human services, criminal justice, or a related field (e.g., medicine, social work, psychology). Minimum of 2 years of experience working directly with individuals facing behavioral health and/or intellectual and developmental disability (IDD) challenges. Job Details
