Responsibilities
The Hospice Aide Under the direction and ongoing supervision of a registered nurse, provides personal direct patient care services as established and delegated in the Plan of Care daily.
- Reports after patient encounter with the Executive Director, or designee, regarding the patient's condition or environment, following the Care Management process on as needed basis.
- Provides assistance with the following ADLs during each patient visit: mobility, transfers, walking, grooming, bathing, dressing, or undressing, eating or toileting.
- Performs incidental household services essential to the patient's health care at home, conducting household services necessary to prevent or postpone institutionalization.
- Participates in QAPI (QUALITY ASSESSMENT & PERFORMANCE IMPROVEMENT) plan or process identifying problems and reporting administrator daily or as needed.
- Must have a reliable vehicle/transportation.
The Care Coordinator for Hospice with LHC Group is the preferred post-acute care partner for hospitals, physicians and families nationwide, helping patients manage their health at home.
- Helps manage order tracking, IDG preparation, and other tasks associated with patient care management.
- Monitors pending referrals daily and assigns registered nurse or advanced practice nurse for all start of care visits.
- Monitors productivity and distribution of visits and communicates any needed changes with the clinical leader, as appropriate.
- Ensures that the patient visit schedule supports continuity of care and excellence in patient care management by scheduling most nursing visits to the patient's designated RN Case Manager, Social Worker,
- Participates in patient care team meetings to assist with facilitating timely care, via the scheduling process, to patients suffering from symptom crises, transitioning to end of life, or who are experiencing other immediate physical, psychosocial, or spiritual care needs.
- Assists with coordination, scheduling, and tracking of all visits to ensure compliance with all hospice policies and state and federal regulatory requirements.
Education and Experience
License Requirements
- Successful completion of a competency evaluation prior to patient care.
- Current CPR certification requirements.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
Skill Requirements
- Mature and able to deal effectively with the demands of a hospice aide position.
- Must be able to read, write, and carry out directions promptly and accurately.
