Sign on Bonus Available!!! $7,500 greater than 1yr of experience $5,000 less than1yr of experience $2,500 Relocation Bonus (greater than50 miles) (external candidates only) Responsibilities include the coordination and provision of direct and indirect patient care using the nursing process to meet the physical, psychosocial, environmental, and spiritual needs of Carle Home Services specific patient populations and families throughout the geographical area. Telephonically triages incoming home care and hospice calls using the nursing process. RN will assess, evaluate, prioritize, and manage patients' symptoms, treatments and health concerns based on level of urgency and follow established home services guidelines for symptom management. Call the assigned home services staff when a home visit is necessary and provide report of care. Provide home visits as necessary. Qualifications Certifications: Basic Life Support (BLS) Instructor - American Heart Association (AHA); Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR), Education: Associate's Degree; Nursing, Work Experience: Responsibilities Establishes nursing standards, assesses health needs, and plans and evaluates care with patients who have complex health problems.Uses specialized knowledge and skills for patient care supervision.Serves as resource within the organization and/or community.Provides and facilitates competent health care of patients and families in area of practice. Ensures adherence to established standards of care and scope of practice.Primarily provides telehealth triage for acute needs of home care and hospice patient populations for the Carle organization.Documents all patient/family care clearly, concisely and timely in accordance with established policies and procedures, state and federal regulations and Accrediting Organization accreditation standards.Meets with leadership team on a regular basis to report trends and to further develop program.Establishes nursing standards, assesses health needs, and plans and evaluates care with patients who have complex health problems. Compensation and Benefits The compensation range for this position is $32.39per hour - $55.71per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
