General Summary of Position

MedStar Health is currently seeking an Inpatient Case Managerto join our team atMedStarGeorgetown University Hospital. This positionfacilitatesthe delivery of quality, cost effective, patient-centered care from pre-admission through post-dischargetimeframe.

Asa member of the Case Management team, you willmonitorthe care and services delivered to selected patient populations during the acute hospital stay, promoting effective case management andutilizationof resourcesfacilitatingthe continuum of care to achieveoptimalclinical outcomes. Additionally, you will collaborate with multidisciplinary health care teams to develop and coordinate plans of care designed to meet individualized patient outcomes.

Primary Duties:

  • Demonstrate the ability to develop a plan of care that addresses needs across the continuum; develop long and short-term goals with specific time frames for resolution; identify specific services to be provided in the care plan; include the family/caregiver in the plan of care; and show life planning contingencies such as power of attorney and/or advance directives.

  • Collaborate with and complete referrals toappropriate communityagencies forassistancebased on patient need. Initiate referralsin a timely manner. Utilize resources to helppatientresume life in the community and/or to adjust to lifestyle changes.

  • Facilitate communication between the patient and family and the healthcare team. Assess and communicate the patient's social, cultural, emotional, and economic problems to medical staff and other healthcare providers as needed. Participate in multidisciplinary rounds and collaborate with internal and external healthcare providers,patientandfamilyto develop a comprehensive discharge plan, as necessary.

  • Complete psychosocial history or socioeconomic assessmentin a timely mannerper policy standards when referred by healthcare team or asdeterminedby high-risk indicators.

  • Develop andmaintaininformation on community resources and referral requirements based on patient population asidentifiedin the unit scope of practice.

Qualifications:

  • Master's degree insocialwork from a school accredited by the Council on Social Work education

  • 1 - 2 years' experience in social work, preferable in a hospital setting

  • 1 - 2 years' experience in care/case management preferred

  • LCSW-C - Licensed Certified Social Worker - Clinical License in the District of Columbia or the State of Maryland depending on work locationORLICSW - Licensed Independent Clinical Social WorkerLicense in the District of Columbia or the State of Maryland depending on the work location

  • CCM - Certified Case Manager preferred

This position has a hiring range of $72,758 - $130,041
