RN - Case Management

We are seeking a dedicated professional with expertise in case management and utilization management to join our team. This role requires a local candidate with a strong background in hospital/inpatient care management.

• Minimum of two years of experience in case/resource management and utilization management is required.
• Experience in hospital/inpatient care management is essential.
• Preferred candidates will have 3-5 years of Care Management experience.
• Proficiency with EPIC systems is required.
• PRI (Patient Review Instrument) and SCREEN experience or certification is ideal.

• New York State RN License, Primary Source Verification.
• American Heart Association Basic Life Support (AHA BLS) certification.

Monday-Friday, 9-5 schedule.

This position is available due to FMLA/LOA.
