PRN Physical Therapist hospice

Summary

Using discretion and independent judgment, the Physical Therapist provides age-appropriate services for hospice, including the following: comprehensive assessment, direct treatment to individuals, development of a home exercise program, patient/family instruction, participation Interdisciplinary Group, evaluation and selection of adaptive equipment, and documentation of services according to hospice standards.



Responsibilities

  • Initiates physical therapy evaluation according to physician orders and Plan of Care for the purpose of symptom control or to enable the patient to maintain activities of daily living and basic functional skills on a daily basis.
  • Evaluates the patient to determine physical therapy needs, establish goals and determine treatment plan on a daily basis.
  • Documents in the patients clinical record each visit made to the patient and gives report including patients reaction to treatment and any changes in the patients status including comfort level and follow-up to physician and Administrator using the Care Management process on a daily basis.
  • Participates in the Interdisciplinary Group and provides applicable input for the patients Plan of Care as needed.
  • Provides services within the scope of practice as defined by state law governing physical therapy, in accordance with the Plan of Care, and in coordination with other members of the Interdisciplinary group on a daily basis.
  • Instructs and informs participating members of the Interdisciplinary group, the patient, the physician and the family functional limitations and progress towards goals as defined by the Plan of care on a daily basis or no less than two weeks time period.
  • Prepares a written discharge summary when physical therapy services are discontinued with a copy retained in the patients clinical record and a copy forwarded to the patients physician conducted upon discharge from the physical therapy service.
  • Maintains strict patient confidentiality on a daily basis at all times.
  • Instructs, when appropriate, patient, family and other members of the hospice team in certain phases of physical therapy in which they may work with the patient.
  • All other duties as assigned.



Education and Experience Education Requirements

  • Recent graduates who have applied for the registration exam may be considered for provisional


License Requirements



  • Must be currently licensed to practice physical therapy in the state of practice.
  • Current CPR certification
  • Must have a current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

  • AL, AR, AZ, CA, CO, FL, GA, ID, IL, IN, KS, LA, MI, MN, MO, MS, NC, NV, OH, OK, OR, PA, SC, TN, TX, VA, WA, WI, WV: No additional state specific requirements.


Skill Requirements

  • Membership or certification in national association (as applicable)
