Saint Therese of Oxbow Lake is looking for a Director of Social Services to join our dedicated team! Since 2006, Saint Therese of Oxbow Lake has ensured seniors have every opportunity to enjoy a rewarding lifestyle. With 142 independent living, assisted living, memory care apartments and 64 long-term and transitional care suites, our Brooklyn Park campus is a vibrant, established community where it's easy to establish positive connections. This position is full-time and will typically work Monday-Friday.

A Director of Social Services is responsible for:



  • Provide leadership and guidance to the department.
  • Provide support and advocacy for residents and/or families.
  • May assist in admission and discharge planning for the community.
  • Will manage a resident caseload, includng assessments.
  • Provides direction and support to social workers, management, and other staff.
  • Develops and administers Social Services related policies and procedures.
  • Manage department budget.
  • Work in collaboration with Admissions staff and nursing team.
  • Attend Discharge meetings as requested.


    Qualifications:



    • Bachelor's degree in Social Work , Sociology or related field.
    • Licensed Social Worker in the State of Minnesota (preferred).
    • Minimum three-five years social work professional level experience.
    • Previous experience with discharge planning is preferred.
    • Two years management or supervisory experience highly desirable.
    • Proficient in computer software/EMR documentation.

    • Demonstrated working knowledge of Social Services department functions and related Federal and State regulations



    • Demonstrated abilities to participate in and collaborate with groups



    • Ability to positively manage and communicate change and to model appropriate behaviors to staff and others



    • High level of commitment to and success with effective customer service skills



    • Demonstrated leadership abilities



    • Effective time management/prioritization and problem solving skills



    • Excellent written and verbal communication and presentation skills

    Salary Range: $65,000 - $87,750, based on experience
