Job Summary:

The Service Coordinator/ Group Facilitator- Team Leader is responsible for the delivery of direct services to the clients and families specifically facilitation of group and recreational sessions as designated by the Community Youth Development Program. The Team Leader is responsible for supervising four service coordinators/group facilitators while ensuring each staff has equal workload, assign service sites, ensure data entry and file completion. Additionally, the Team Leader will identify community sites in the service areas to offer program services, coordinate meetings and establish collaborations or partnerships. The Team Leader represents Southwest Key Programs in the most positive manner with prospective, former and current youth/families, funding sources, suppliers and the community served. The Group Facilitator/ Team Leader maintains a flexible, organized and efficient work schedule and is subject to work more than a normal work day and be on-call to facilitate program services.



Essential Functions:

    Recruit and enroll eligible clients in programming.
  • Complete intake and dismissal paperwork, surveys, and other as needed.
  • Facilitate weekly group sessions out in the community using the designated curriculum.
  • Facilitate and/or co-facilitate group and recreation sessions.
  • Create thorough lesson plans and ensure all aspects of the lesson plans are delivered in a timely manner.
  • Ensure curriculum is delivered professionally and effectively to meet desired learning objectives.
  • Maintain attendance records of all clients attending group sessions and workshops.
  • Enter group notes and case notes using ETO internal database system.
  • Ensure clients participate or are offered all components or services offered at the program.
  • Provide case management services as needed including referrals and linkages.
  • Provide advocacy specified by Southwest Key Programs and contract guidelines.
  • Regular and punctual attendance.
  • Work within the community to promote and engage Southwest Key Programs and organize or attend events as needed.
  • Ensure service coordinators enter all daily documentation in SWK & funder database in a timely and efficient manner.
  • Ensure service coordinators maintain physical and web-based client files.
  • Ensure service coordinators meet monthly and annual target numbers.
  • Assign and monitor all service coordinators service sites; ensure coordinators have up to date lesson plans, sign in sheets, and materials needed.
  • Assign and monitor team members work in Southwest Key Programs database to ensure outcomes and data are meeting program model expectations.
  • Identify community sites to offer program services in and coordinate outreach & recruitment efforts.
  • Develop and maintain productive relations with various community agencies to assist with meeting target goals.
  • Maintain positive working relationships with community partners, attend meetings as needed.
  • Provide leadership, supervision, and modeling to all employees, volunteers and/or contractors.
  • Adhere to and implementation of the terms of the contract in accordance with the funding source and Southwest Key's Programs policy and procedures.


Other Functions:

  • Maintain all required training and certifications.
  • Attend and participate in all required professional trainings and workshops.
  • Maintain open and consistent communication with Program Director or designee and other team members.
  • Refer all problematic work performance in accordance with company policy, to the Program Director.
  • Initiate and maintain effective communication with all program staff members.
  • Able to react to change productively and handle other essential tasks as assigned.
  • Assist with the interview & hiring process of Service Coordinators.
  • Works well independently and as an effective team member.
  • Well organized, meticulous, and thorough in work habits.
  • Work evening and weekend hours.


Qualifications and Requirements:

  • A Bachelor's Degree in social service or a related field with Two (2) years of experience working with at risk you the and their families; may include part time, volunteer or internship experience.
  • Experience working with at risk population
  • Computer literate with working knowledge of Microsoft Windows (Microsoft Word, Excel), variety of online database systems.
  • Possess good verbal and writing skills.
  • Bilingual in Spanish and English
  • Clear background check from appropriate entity
  • Must possess a valid state driver's license, registration and maintain state mandated car insurance


Physical Demands:

Must be able to obtain First Aid Certification (CPR). Must be able to verify client's whereabouts by driving, sitting, standing, and walking for extended periods of time. Daily use of various office equipment. Bending, stooping, climbing up stairs and lifting up to 15 lbs. required to complete daily tasks.



Work Environment:

This is a program which requires daily contact at various client locations. Work shifts are subject to change. Noise levels vary from mild to moderate. This position may require extended periods of time in an automobile. Exposed to prevalent weather conditions.
