Your role with us

As a Master of Social Work (MSW) care manager in psychiatric patient services, you will provide essential psychosocial support and case coordination for adults receiving inpatient psychiatric care. In this highly collaborative role, you'll partner with an interdisciplinary team to develop and implement individualized treatment and discharge plans that address each patient's emotional, social, behavioral, and substance use needs.

What you'll do

  • Complete comprehensive psychosocial assessments within 72 hours of admission and perform Mini-Mental Status Exams (MMSEs) per provider orders.



  • Develop patient-centered discharge plans that account for family dynamics, emotional wellbeing, safety needs, and readiness for community reintegration.



  • Coordinate care team efforts to ensure that treatment plans are integrated, multidisciplinary, and responsive to patients' evolving psychiatric and psychosocial needs.



  • Engage with patients, families, and outside providers to support continuity of care, including connections to community-based services and substance use programs.



  • Serve as a liaison with the unit discharge planner and external agencies to align discharge resources with the patient's clinical progress and social situation.



  • Document all assessments, interventions, treatment planning activities, and group therapy participation clearly and promptly in the electronic health record (EPIC).



  • Facilitate patient and family understanding of mental health conditions, treatment recommendations, and available support systems to enhance treatment engagement and compliance.



  • Lead therapeutic groups as assigned, using professional standards for group facilitation within psychiatric settings.



  • Identify and report suspected abuse or neglect in accordance with Oregon law, acting swiftly to protect vulnerable patients.



  • Contribute to a safe and therapeutic environment by completing required Managing Aggressive Training (e.g., SAVE) and applying de-escalation techniques as needed.

Required qualifications

  • Master's degree in social work from the council on social work education (CSWE) accredited school.
  • Licensed master social worker (LMSW), licensed clinical social worker (LCSW) or certified social worker associate (CSWA), pursuing LCSW, required by State of Oregon.
