Operates the department delivery vehicle in a safe manner to ensure the timely documented, accurate and organized handling of its contents. 1 .1 Loads and unloads hospital vehicle as required, to ensure all materials being transported arrive in a safe manner as observed by manager. 1 .2 Drives Hospital vehicle in a safe manner obeying all Connecticut state rules of the road, as observed by manager. 1 .3 Maintains Connecticut state driver's license with no violations, as observed by annual license check with Connecticut Motor Vehicle Department. 1 .4 Maintains accurate driver log daily, documenting trip times (arrive/depart), rack truck totals, route slip numbers (Stores/Receiving/SPD) as observed by manager. 1 .5 Calls dock prior to delivery trips to ensure communication of detailed contents of truck, approximate arrival time and coordination of delivery as observed by manager. 1 .6 Maintains a consistent schedule of delivery trips to ensure consistent, smooth, and timely flow of materials and equipment as observed by manager. 1 .7 Transports mail to and from the materials center, maintaining accountability of all materials handled, as observed by manager. 1 .8 Separates all incoming materials and places material in appropriate areas as observed by manager: 2. 2. Assists with the monitoring of all transportation activity to ensure accurate data is available. 2 .1 Documents daily delivery log sheet to ensure complete statistical input is provided as observed by manager. 2.2 Files and maintains file daily of all signed route slips to ensure accountability of all material delivery, as observed by manager. 3. 3. Follows established safety procedures in the performance of all duties to ensure a safe environment. 3 .1 Operates department delivery vehicle, propane fork truck, and electric pallet trucks in a safe manner as identified in initial orientation with no more than 2 non serious violations in this evaluation period as observed by manager. 3 .2 Maintains materials center equipment and departmental delivery vehicle cab and cargo compartments in a neat and orderly operating condition, as observed by manager. 3 .3 Fills electric vehicle battery water levels. 3 .4 Replaces empty propane tank on fork truck as required. 3 .5 Maintains truck bay, free of all debris. 3.6 Coordinates repair of all malfunctioning parts of departmental delivery vehicle with repair vendor, as required and notifies manager of all malfunctions, pertaining to departmental delivery vehicle. 4. 4. Performs various departmental related duties as assigned by Manager to contribute to the efficient operation of the department. 4 .1 Verifies incoming materials, replenishes shelves to maintain accurate materials available for distribution, and reports variances to Quality Assurance Clerk and Manager. 4 .2 Reports variance: Unit of measure, item description, MFG packaging, quantity ordered, and material condition. 4 .3 Performs cycle counts as required, to maintain accurate inventory totals as observed by manager when applicable. 4 .4 Courteously responds to user phone requests for Material Center service follow up as observed by manager. 4 .5 As requested, works overtime hours and/or adjusted scheduled hours in order to provide necessary staffing levels to accommodate workday 90% of the time as observed by manager. 4.6 Assists in the Annual Inventory as required, to provide adequate staffing throughout the annual inventory period as observed by manager. 4.7 Fills in for other Logistical or Receiving personnel as directed by manager or designee. Qualifications EDUCATION High School Diploma or Equivalent EXPERIENCE One year experience driving a van and a clean driver record. Up to one (1) year experience driving a truck having a body length of 20 feet or longer. This position includes or may include operating a motor vehicle intrastate, with GVWR of 18,001 lbs. or more. In this position you must be at all times compliant with all relevant Department of Transportation regulations. Compliance requires completion of, and provision of documentation including: o Successful passing of Road Test, as directed by YNHHS, in accordance to and compliance with DOT regulation. YNHHS will provide the testing opportunity. If a Commercial Driver's License (CDL) is held by applicant and current, YNHHS Road Test is not necessary. o Current and valid driver's license (Must be at least 21 years old with no active suspensions) o Current and valid Medical Examiner's Certificate, issued by a practitioner on the Federal Motor Carrier Safety Administration (FMCSA) National Registry. o If you have a physical impairment including hearing loss that affects your ability to operate a Commercial Motor Vehicle safely, you must obtain a Skill Performance Evaluation (SPE) certificate. The SPE Certificate is obtainable through application with YNHHS to the Division Administrator of the FMCSA . o Be willing, upon hire, to provide requested documentation, including: o Annual Driver's Certification of Violations form (honestly completed & signed). o Medical Examiner's Certification Documentation, including but not limited to Medical Examiner's Certificate, may need to be updated frequently. The Driver's Certification of Violation's form must be completed every 12 months throughout employment. Applicant is in agreement with updating as often as required for organization to meet compliance. YNHHS will review Motor Vehicle Report, reflect your documented driving experience and any violations. YNHHS must be in agreement that your record reflects acceptable driving competency. SPECIAL SKILLS ' Possesses a valid public service driver's license. Meet all qualifications required by the state department of motor vehicle or any other Regulatory agency involved in the transportation of people. Undergo a medical criminal and drivers record review as required by the state department of motor vehicles. ' Ability to operate a variety of material handling equipment. Must have a valid Connecticut driver license (Class: D). Must possess basic math skills. PHYSICAL DEMAND Driving most of the day. Exposure to all weather conditions; heat, cold, rain and snow. Must be reasonably fit and healthy, without any back, heart, or neurological conditions. Need to have good hearing, reflexes, hand-eye coordination, and eyesight (with or without glasses).
