Social Work Assistant – Mobility Specialist

$5,016 - $6,096/Monthly*

Starting salary up to $35.05/hour* ($73,153/year) plus a competitive total compensation package!  

This recruitment is currently being conducted to fill one time-limited Social Work Assistant - Mobility Specialist position in the Human Services Department's Adult and Aging Division, which is currently funded through 12/31/2026.

The Social Work Assistant - Mobility Specialist in the Adult and Aging Division of the Human Services Department is responsible for assisting clients with personalized assistance to access transportation resources, which may include completing and submitting applications for Medi-Cal, IHSS, and paratransit. They will aid in coordinating services, as directed by the Aging and Disability Resource Unit (ADRU) Supervisor, including communicating with other transportation and social service providers throughout the county and community. Additional responsibilities include:

  • Coordinating and providing transportation resources for clients and families
  • Identifying and reporting on gaps in resources and areas of need
  • Updating and maintaining lists of new and existing transportation resources and agency contacts
  • Completing accurate and timely case documentation of client contacts and work performed
  • Attending bi-monthly ADRU program and county transportation meetings and participating in required trainings

The ideal candidate will possess many of the following:

  • Education in community outreach, psychology, social work, or related human behavior fields
  • Experience in social services, particularly working with older adults or people with disabilities
  • The ability to interact effectively with a diverse client base while respecting their autonomy
  • Experience conducting home visits and working with clients with functional support needs
  • Knowledge of paratransit and public transportation services
  • Strong organizational skills and the ability to manage multiple tasks for different clients in a fast-paced environment
  • Proficiency in Microsoft Office Suite, including Word, Excel, and Outlook and a basic understanding of Zoom, Teams, or similar virtual communication platforms
  • Strong verbal and written communication skills, with the ability to present in group settings and meetings
  • A passion for public service and making a positive impact on the lives of community members
  • Bilingual (English/Spanish) skills are highly desired but not required
