Patients treated in the Termeer Center are often on a Phase 1 clinical trial which presents many challenges for the average patient. Phase 1 trials require frequent appointments, sometimes multiple times a week and for long hours at a time, including biopsies and scans, rigid treatment schedules and auxiliary appointments dictated by the protocol. Patients often present with complicated social needs such as transportation, VNA needs, FMLA or disability paperwork requirements, and language and/or health literacy barriers. The Case Manager for the Termeer Center will work with the existing team members in Termeer (Physicians, Clinical Research Coordinators, Research Nurses, Infusion nurses and PCAs) to do a thorough needs assessment of each new Phase 1 patient and then oversee the various resource and interdisciplinary referrals, follow-up on SW, Palliative Care and supportive care referrals, facilitate prior authorizations, protocol reimbursements, challenging insurance co-pays, complicated billing issues and overall support Termeer patients on clinical trials. A dedicated Case Manager to the Termeer Center would be expertly equipped to help the patients who go on a Phase 1 trial to face the unique challenges of these trials.
Case management responsibilities include but are not limited to: *Needs assessment on intake of new patients in screening or on C1D1 *communication with the interdisciplinary team for patients and their caregivers *Coordination with referring oncologist at MGH or outside *crisis intervention and care coordination, information and safety planning *referral to complementary therapies with SW, integrative therapies, Palliative Care, PT and OT, *support resource referral and obtaining unique resources for patients in need, *navigation and application for any funding resources available to specific cancer diagnosis *Assist with transportation issues (PT1 through Mass Health, the Ride, parking at MGH, the shuttle to and from OCB, etc.) *Setting up rides for patients using Uber Health *processing of Prior Authorizations *oversee reimbursements offered by protocols *FMLA and disability paperwork processing *Communication with Patient Financial Services for patients who have difficulty with insurance co-pays or if a pt has no insurance to cover some costs *Coordinating with Lay Navigators or Nurse Navigators when one is available *assisting with obtaining growth factor stimulants (ie: Granix) *tracking down prescriptions or medications that should have been delivered *Setting patients up for home oxygen or home infusion and other home care services *Training and sharing information with existing Cancer Center Case Managers *Help patients transition to hospice obtain home services Additionally, this individual will attend team meetings, develop educational curricula for the Research team and interface with Phase 1 physicians, APPs, Research nurses and infusion nurses, CRCs, CCPO Finance team members and other members of the Research Team. This position will also be responsible for tracking the number and source of referrals and, in collaboration with the director, for evaluating the success of the supporting Phase 1 patients.
