As a PHSO Resource Coordinator, you will play a pivotal role in coordinating care for our primary care population. Your responsibilities will include:
DEPARTMENT Population Health Services Organization (PHSO), Quality Team QUALIFICATIONS Education: Associates Degree in Human Services or 2-3 years of experience in a healthcare setting (or equivalent). Bachelor's level degree in Human Services preferred. Experience: Care coordination or other transferable skills preferred. Customer service preferred. Skills: We are looking for a proactive and reliable candidate who requires minimal oversight. Strong communication skills are essential, and applicant must be comfortable making and receiving phone calls. It is important for the applicant to be collaborative in nature and excited to become a part of a cross-functional team.
