Requirements

The ideal candidate will possess the following qualifications:



  • Bachelor's Degree, psychology, sociology, social work, nursing, rehabilitation, pre-med, theology or anthropology OR

    • Bachelor's Degree in any other subject matter and have 12 college level semester hours in humanities or social services plus 1 year of experience in a CASSP system program and be enrolled for certification by OMH as a mental health family-based workeR


  • 1 - 3 years in a Child and Adolescent Service System Program (CASSP).
  • A valid Pennsylvania Driver's license and access to reliable transportation is required.
  • Ability to work within, collaborate with and clearly communicate within an interdisciplinary team
  • Ability to work flexible hours, including evenings and weekends, in order to meet the needs of children and families on caseload.
  • Preferred key competencies: Crisis intervention, Case management, Documentation, Therapy facilitation/co-facilitation

We are seeking a Family Based Case Manager to join our team at our program in Kennett Square, PA.

Position Details



  • The Family Based Case Manager (FBCM) coordinates case management and linkage to community programs and services.
  • FBCM also co-facilitates family therapy sessions, parent coaching, and crisis intervention for each child assigned to his/her caseload. Clinical sessions are primarily delivered in the community (client homes, schools, etc.), case management can be in an office environment or community
  • FBCM provides treatment services to families, including skills training and education designed to enable families to care for their children with mental health needs at home.
  • The FBCM maintains regular communication with families, schools, physicians, and other service providers involved with the child and family.
  • The FBCM works flexible hours, including evenings and weekends, to meet the needs of children and families on their assigned caseload and travels to client's homes, other agencies, and community locations for purpose of arranging and providing community and home-based services for children, adolescents, and families.
  • This position requires rotating on-call coverage.


Earn: $24.00 an hour
