SUMMARY The Nurse Practitioner works collaboratively with the health care team. He/she will assess, plan, implement and evaluate care for patients. He/she will, in collaboration with other team members provide programs, treatment and counseling appropriate to the patient's plan of care. He/she will consistently demonstrate proficiency in working with the designated patient population and serve as a unit resource and teacher. EEO/AA/Disability Neteran.
Responsibilities
1 . Accountable for the delivery of quality patient care utilizing a therapeutic process. 1 .1 Conducts individual exams, evaluation and treatment to patients. 1 .2 Leads and facilitates groups as appropriate, 1 .3 Acts as a resource to staff and physicians. 1 .4 Reassesses the needs of the patients at regular intervals to provide necessary support. 1 .5 Coordinates care of patients through appropriate collaboration with physicians, nursing staff and ancillary departments.
- Evaluates outcomes of patient care
- Identifies problem prone practices and initiates solutions to maximize desired outcomes.
- Works collaboratively with Quality Management in developing PI studies and measuring outcomes.
- Evaluates outcomes and identifies improvements in care.
Performs and coordinates documentation audits to assure compliance with standards.
- Acts as a role model
- Ensures that staff is familiar with current theories and practices.
- Identifies gaps in knowledge and barriers to practice. Works collaboratively to plan programs designed to update staff.
- Provides Staff development as necessary.
- Attends mandatory hospital/unit based in-services
- Identifies own goals and plans for goal achievement consistent with the department's tong and short-range goals.
- Actively participates in own performance appraisal.
- Demonstrates the ability to accept constructive criticism by showing improvement within established time.

- Shares information gained at educational programs with appropriate personnel.
- Speaks to groups and provides other hospital staff with information to share at classes and lor conferences.
- Meets regularly with physicians to assure that appropriate information is in their offices and that they are up to date on current practices.
- Develops materials that can be used in the community to assist patients and promote the program.

- Works collaboratively with the Medical Director to establish an efficient and patient friendly environment.
- Utilizes effective time management skills. Organizes day efficiently and meets organizational deadlines.

- Meets volume parameters of the program.
- Maintains current knowledge of regulatory requirements and participates in meeting JCAHO, State and other code/requirements.
- _ Responsible for promoting the Greenwich Hospital Patient Guest Relations program.
- Assures that all patients are treated with respect and that needs are met promptly.
- Shows consideration for patients family and visitors by acknowledging them promptly and speaking in a way that shows concern
- Remains calm in stressful situations promoting a sense of confidence to patient and family members
- The Nurse Practitioner must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patients requirements relative to his or her age-specific needs, and to provide the care needed as described in the unit's/department's policies
- Triage obstetric patients to include history and physical exam, assess fetal monitoring, order and review of laboratory and radiologic testing. Review test results with obstetrician.

- Admit obstetric patients for induction of labor and cesarean birth.
- Admit antepartum patients with complications of pregnancy to include suspected preterm labor, PPROM, and preeclampsia. Collaborate with Laborist and Maternal Fetal Medicine service.
Il . Admission of postpartum patients with postpartum hypertensive complications in collaboration with Laborist -
- Round on postpartum patients to include coordination of care and discharge planning.
- Assess antepartum and postpartum patients with complications to include severe hypertension, postpartum hemorrhage and other 0B emergencies.
- Attend labor delivery and post-partum huddles.
15 . Assess fetal monitoring and review plan of care with nurse and obstetrician. 16. Triage obstetric and gynecologic patients presenting to emergency department.
Qualifications
EDUCATION (number of years and type required to perform the position duties): MSN Required, Graduate of Nurse Practitioner Program. Current CT APRN License. EXPERIENCE (number of years and type required to meet an acceptable level of performance): 3-5 years experience in area of specialty preferred. APRNs, PAS, CRNAs and CNMs are required to be appointed to the Affiliated Medical Staff of a YNHHS Hospital and credentialed through the Medical Staff process. Evidence of current competence to practice as {an APRN, PAI CRNA, CNM} via appropriate reference letters from physicians and other practitioners must be able to be obtained during the Medical Staff appointment process in order to be eligible. SPECIAL SKILLS: Point of care ultrasound, Biophysical profile, assess fetal position, interpret non stress tests and external fetal monitoring ACCOUNTABILITY (how this position is held accountable for such as goals achievement, budget adherence, or other areas of accountability): In personal and job-related decisions and actions, consistently demonstrates the values of integrity (doing the right thing), patient-centered (putting patients and families first), respect (valuing all people and embracing all differences), accountability (being responsible and taking action), and compassion (being empathetic).
