Nurse Practitioner Primary Care

Summary:
Under general supervision of the Medical Director Attending Physician in collaboration with RIH Adult Ambulatory Services Senior Leadership the Nurse Practitioner is responsible for evaluation of and development of treatment plans for the care of ambulatory patients. Assists in the care and management of patients to include but not limited to history and physical ongoing assessment and intervention coordination of care post emergency room visits or hospital discharge. All essential functions performed in this position reflect the age specific needs of the patient. Fosters patient and family centered care and nursing services with Brown University Health*s mission vision values with consideration of the life span including all development stages as well as cultural patterns.

Responsibilities:

Demonstrates a fundamental grounding in nursing theory and practice with mastery of knowledge within a defined content area.

Manages a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care over time and across healthcare settings.

Adheres to established clinical session requirements as defined by the Director of Clinical Operations in collaboration with the Medical Director.

Provides direct care to patients in the ambulatory setting.

Demonstrates advanced physical and psychosocial assessment skills and knowledge of family dynamics.

Provides leadership in the development validation implementation and evaluation of standards policies procedures and critical paths to support the patient care delivery model.

Performs direct patient care at the outpatient offices in accordance with State licensure and Federal rules.

Performs phone triage for patient problems/questions for patients under his/her care and for other patients as requested by physicians.

Oversees care coordination in the ambulatory care settings as feasible.

Assists the healthcare team to achieve mutually agreed upon healthcare goals by providing necessary support education and encouragement.

Performs advanced practice skills under the supervision of the Medical Director.

Provides expert consultation for clinical issues promoting increased knowledge and skills for the consultee.

Participates in the education clinical research and mentoring of healthcare providers.

Facilitates patient/family education in area of expertise.

Annually completes mandatory training as assigned

Prescriptive privileges per licensure.

Other information:

Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursingorlicensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.

BASIC KNOWLEDGE:

Masters of Science Degree Graduate of accredited Nurse Practitioner program. Specialty certification required.
Baccalaureate in Nursing
Certified in Basic Cardiac Life Support (BLS).
Bilingual Spanish Speaking preferred.
Demonstrates knowledge and skills necessary to provide care to patients with consideration of aging processes human development stages and cultural patterns in each step of the care process.

EXPERIENCE:

Two years related experience as RN in acute care hospital or ambulatory care setting and 2 years* experience as a nurse practitioner require

WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:

General hospital environment with occasional stressful conditions associated with patient care.

SUPERVISORY RESPONSIBILITIES:

None.
