Summary: Responsibilities: Demonstrates a fundamental grounding in nursing theory and practice with mastery of knowledge within a defined content area. Manages a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care over time and across healthcare settings. Adheres to established clinical session requirements as defined by the Director of Clinical Operations in collaboration with the Medical Director. Provides direct care to patients in the ambulatory setting. Demonstrates advanced physical and psychosocial assessment skills and knowledge of family dynamics. Provides leadership in the development validation implementation and evaluation of standards policies procedures and critical paths to support the patient care delivery model. Performs direct patient care at the outpatient offices in accordance with State licensure and Federal rules. Performs phone triage for patient problems/questions for patients under his/her care and for other patients as requested by physicians. Oversees care coordination in the ambulatory care settings as feasible. Assists the healthcare team to achieve mutually agreed upon healthcare goals by providing necessary support education and encouragement. Performs advanced practice skills under the supervision of the Medical Director. Provides expert consultation for clinical issues promoting increased knowledge and skills for the consultee. Participates in the education clinical research and mentoring of healthcare providers. Facilitates patient/family education in area of expertise. Annually completes mandatory training as assigned Prescriptive privileges per licensure. Other information: Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursingorlicensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing. BASIC KNOWLEDGE: Masters of Science Degree Graduate of accredited Nurse Practitioner program. Specialty certification required. EXPERIENCE: Two years related experience as RN in acute care hospital or ambulatory care setting and 2 years* experience as a nurse practitioner require WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS: General hospital environment with occasional stressful conditions associated with patient care.
