Job Description

Responsibilities:

The advanced practice provider (Nurse Practitioner/Physician Assistant) within the ambulatory setting works in collaboration with, and is an integral member of, a multidisciplinary health care team. The APP provides comprehensive care in the Urology ambulatory practice.

Assessment & Management

  • Performs complete history & physicals and documents in the medical record.
  • Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated.
  • The APP may serve as the first responder for both nursing and house staff in the event of patient status changes, orders, and in situations requiring procedural and resuscitative interventions.
  • Assesses for risks associated with the care of the acute and complex chronically ill patient including: medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures.

Diagnosis

  • Collaborates with interdisciplinary health care team in making diagnoses of acute and complex chronic conditions.
  • Manages diagnostic tests through ordering, interpretation, performance, and supervision.
  • Formulates differential diagnoses by priority.
  • Diagnoses complications and orders appropriate interventions

Formulates Plan of Care

  • Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address complex acute and chronic health care needs.
  • Utilizes evidenced based practice guidelines and protocols in an individualized, dynamic plan of care that can be applied across the continuum.
  • Implements and modifies plan of care.
  • Prescribes diagnostic strategies and therapeutic interventions.
  • May perform advanced procedures consistent with privileges and competency validation.

Documentation

  • Documents/dictates key components of patient's progress via daily progress note, transfer, and discharge summary, and or clinic note where applicable. (h&p, daily progress notes, plan of care, problem lists, procedure notes, acute event note, discharge summaries, in medical record per specific patient unit or service, death summary)
  • Documentation is timely, meets acute care compliance standards and captures patient acuity.

Communication and Collaboration

  • Maintains ongoing communication and collaboration with interdisciplinary health care team.
  • Facilitates and communicates with patient, family and staff to promote continuity of care across the inpatient and outpatient continuum of care.

Professional Practice

  • Demonstrates Professional Practice behaviors including: preceptor/mentoring, education and instruction of graduate students.
  • Seeks opportunities for active engagement in research and the analysis of evidenced based practice.
  • Actively participates in Advance Practice Grand Rounds and specialty specific meetings.
  • Maintains CEUs, and membership in a professional organization.

The Surgery Advanced Practice Clinician works within the department of Surgery, supporting the program by covering all aspects of the surgery program. The APC is part of a team that includes their colleagues, attending physicians, and residents. The APC will provide all aspects of clinical care for patients throughout the continuum. This includes, but is not limited to the Emergency Room, inpatient hospital and Operating Room. The surgical APC interacts with various disciplines within the hospital including consultants in the hospital in an effort to coordinate care. Coverage will be provided at Jefferson Lansdale Hospital (JLH).

  • Attend morning sign-out with the residents & attendings as a part of the surgery team.
  • Manage surgery patients to include the following documentation: history & physicals, order placement, brief operative notes, progress notes, discharge summaries.
  • Prescribe medications including narcotics under Federal & State prescribing laws.
  • Perform surgery consults.
  • Perform evaluations in the Emergency Room.
  • Attend unit based safety briefings when indicated
  • Provide preadmission testing communication with elective surgery patients.
  • Collaborate with preadmission testing nurses regarding elective surgery patients.
  • Collaborate with nurses and ancillary staff from various areas within the hospital.
  • Provide OR first assistant coverage; including skills for open, laparoscopic, and robotic procedures.
  • Carry hospital issued phone and address issues, perform consults.
  • Work in collaboration with the resident team to cover the needs of the surgery service line.
  • Round on patient, write progress notes, evaluate laboratory and imaging data.
  • Perform suturing, drain removal, abscess drainage, wound care, skin biopsy, and minor procedures.
  • Attend monthly/quarterly programmatic meetings, in-person or via Zoom
  • Staggered work schedule with coverage until 10pm Monday-Friday
  • Saturday and Sunday full coverage, on average one weekend per month.
  • Holiday full coverage, shared amongst the APC team to include those observed by Jefferson Health:
  • New Year's Day, Martin Luther King Day, Memorial Day, 4th of July, Labor Day, Thanksgiving Day, Christmas Day.
  • Coverage at Jefferson Lansdale Hospital to include patient care and preadmission testing communication.
  • Ensure strong communication among team to include APCs, physicians, floor nursing staff, OR staff and members of other multidisciplinary teams.
  • Efficient use and navigation of the electronic medical record "EPIC" for patient care.
  • Ability to multi-task, prioritize, and triage duties and patient care.
  • Possess strong communication skills.
  • Be flexible and adaptable to change
  • Maintain professional appearance and follow code of conduct as per hospital guidelines.
  • Complete mandatory education yearly.
  • Maintain professional licensures and certifications.
