Healthy Transitions - Case Manager (Full time) The Healthy Transitions team at Community Care Alliance is currently seeking a Bachelors Level Case Manager to provide trauma-informed services throughout the Northern RI communities of Woonsocket, Lincoln, Cumberland, North Smithfield, and Burrillville. Healthy Transitions (HT) team members are a motivated, flexible, diverse group of staff who strive to maintain a genuine understanding of the unique needs of our program participants. HT program participants are young people (16-25 years old) who struggle with serious emotional/ mental health concerns & co-occurring disorders, who have not typically benefitted from traditional models of treatment and services. Priority populations for HT programming include youth who have previously experienced residential placement/ youth aging-out of the foster care system and young people experiencing their First Episode of Psychosis (FEP). The HT case manager works closely with the program Clinicians, Nurse, Recovery/ Substance Use Specialist, and Psychiatrist to provide holistic services that address a variety of biopsychosocial factors impacting each participants mental health, well-being, and recovery. In an effort to provide relevant supports and change the life trajectories of HT participants, Healthy Transitions staff will utilize a variety of team-based treatment modalities to deliver innovative, individualized services, including Coordinated Specialty Care for First Episode Psychosis, and Dialectical Behavior Therapy. Included in the program outcomes are changes in current State policies for transition services for young adults. The ideal candidate has the ability to engage effectively with participants as well as their families, has experience in the mental health field, a general understanding of the agencies and systems that serve young people, and a genuine desire to work with young adults. Case manager must have excellent time management and organizational skills, and the ambition to take on tasks and responsibilities to enhance program quality and efficiency. Individual will have the ability to communicate effectively with team members on a daily basis, is competent in teaching life skills and coping strategies to young people in both individual and group settings, the ability to maintain a somewhat flexible schedule to accommodate the needs of the program participants, including occasional evenings and weekends. Bilingual candidates are strongly encouraged to apply. Education/Licensing Requirements: Bachelor's Degree in Human Service related field required. Previous experience working with youth and/or young adults with mental health issues. Reliable vehicle, auto insurance and valid driver's license.
