Job Summary and Responsibilities

Acentra Health is seeking a Care Coordinator - Healthcare (Remote within Oregon and bordering geographies)
Job Summary:
  • Provide telephonic care coordination outreach to support preventative health activities, including social needs, to reduce barriers to accessing care while optimizing health outcomes.
Responsibilities:
  • Ensures the responsible delivery of comprehensive care coordination services to enrollees.
  • Conducts health risk assessments.
  • Provides critical resources to fee-for-service members in real-time.
  • Maintains metrics for telephonic outreach and member engagement.
  • Participates in team case reviews for collaborative assessment and coordination planning to ensure quality care as needed.
  • Consults with lead, supervisor, and/or manager to ensure the cost-effective and efficient achievement of contract requirements, including service delivery, by reporting accurate and timely work hours incurred during job position duties.
  • Will need to work in a highly dynamic environment, completing assessments with members and coordinating with Oregon Health Authority (OHA), hospitals, and other caregivers.
  • Read, understand, and adhere to all corporate policies including policies related to HIPAA and its Privacy and Security Rules.

** The selected candidate must reside in the State of Oregon or within a 45-minute commutable distance to Oregon. **

** Work Schedule: Monday through Friday from 8:00 AM to 5:00 PM Pacific.


Qualifications

Required Qualifications/Experience:
  • A minimum of a High School Diploma OR GED equivalent.
  • 2+ years of experience working in a healthcare environment.
  • 1+ years of care coordination experience.
  • Knowledge of medical terminology.
Preferred Qualifications/Experience:
  • Medical Assistant or an Associate's degree in a health-related field.
  • Bachelor's degree.
  • 2+ years of Medicaid/Oregon Health Plan (OHP) or other health plan or clinic/hospital experience with knowledge of public health care.
  • 2+ years of experience completing health and/or social needs assessments.
  • Telephonic outreach or call center experience.
  • Ability to work efficiently telephonically while completing system inputs.
  • Bilingual experience.
  • Positive attitude.
  • Knowledge of customer service principles.
  • Strong written and verbal communication skills.
  • Strong field or motivational interviewing skills.
  • Strong computer skills (e.g., care management applications, Internet, Microsoft Office [Word, Excel]).
  • Strong prioritization and organizational skills.
  • Ability to receive verbal and written feedback in a professional manner and implement performance productivity improvements as needed.
  • Ability to interpret real-time historical information to inform and assist members in real time.
  • Knowledge of social needs.
  • Knowledge of regulatory accreditation standards.
  • Knowledge of principles, ethics, and precepts of care management service delivery.
