The Social Work Manager (LCSW):

  • Supervises the social work team ensuring the proper and timely completion of social work assessments, 701B assessments, and Medicaid recertifications per state and federal regulations, training and support of new social workers as well as facilitation of ongoing social work support and education.
  • Evaluates and counsels' individual social workers on an annual and ongoing basis as needed providing feedback and coaching.
  • Provides direction and oversight of the provision of social services to PACE participants and their families.
  • Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
  • Is an active participant in the Interdisciplinary Team and attends IDT, and Plan of Care meetings. Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in, Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
  • Acts as liaison for the Social Work Team with participants, caregivers, and community agencies regarding orientation to and ongoing relations with Interdisciplinary Team, day center, and other PACE staff, including volunteers.
  • Assists with IDT Meeting Facilitation as Needed
  • Provides ongoing support, counsel, and education to participants and family regarding a variety of issues, including but not limited to the aging process, dementia, grief and loss, end of life, disease processes, difficult family dynamics and changing roles, PACE model and PACE health services.
  • Acts as facilitator for meetings with participants, family, caregivers, and community agencies to clarify, or problem solves issues regarding the plan of care. Mediates discussions between all parties.
  • Ensures completion of all appropriate Advance Directives, including Healthcare Surrogate, Healthcare Proxy, and Do Not Resuscitate Orders, by Social Work Team members. Acts as a resource to other team members and day center staff regarding topics such as complex Mental Health issues, Substance Abuse, healthy boundaries, psychotherapy, dementia, difficult behaviors, and difficult personalities.
  • Completes and ensures completion of documentation of clinical service, in participant's medical records including initial assessments, reassessments, change of status, temporary or permanent placements, hospital admissions and discharges, home and nursing home visits and other significant events according to PACE documentation requirements.
  • Tracks AHCA Medicaid renewal dates and annual DOEA Level of Care (re)certification dates, assisting as needed with obtaining and submitting necessary documentation.
  • Guides Social Work Team in development and facilitation of Support Group services for participants and caregivers.
  • Assists participants and caregivers in filing grievances.
  • Assists participants and families in coordination with Enrollment Specialist to keep resources within guidelines for Medicaid eligibility.
  • Acts within scope of his or her authority to practice.
  • Follow all PACE Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
  • Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants, and families.
  • Maintains safe working environment. Follows PACE Care Safety policies and procedures.
  • Participates in and supports Quality Improvement Initiatives.
  • Participate in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
  • Serve as a chairperson or member of the required committees.
  • Initiate and participate in family meetings.
  • Responsible for ensuring the team is always audit-ready by adhering to departmental audit standards and collaborating closely with the quality and compliance teams.
  • Performs other duties as required.

Qualifications and requirements:

  • Education: Master's degree from an accredited school of social work required. Licensed Clinical Social Worker in the state of Florida. Be legally authorized, currently licensed, registered or certified if applicable in the State of Florida.
  • Experience working on a multidisciplinary team in a hospital, nursing home or community-based setting is preferable. 2 years Managerial/Supervisory experience preferred.
  • Must have a valid driver's license, proof of insurance and have means of transportation.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Successfully complete a Competency Assessment upon hire and receive annual competency assessments.
  • At least one year's experience working with the frail and elderly population.
