PQI Specialist

The Children's Home of Poughkeepsie has been caring for children throughout the Hudson Valley for over 175 years! We strive toempower children and families to achieve optimal health and well being. We are currently seeking a PQI Specialist to join our Home Study and Post Release Services (HS/PRS) Program.

Schedule: 40 hours/week, Monday - Friday, 8:30 am - 5:30 pm

Position Summary:

The Performance and Quality Improvement (PQI) Specialist is responsible for oversight as it relates to documentation, compliance and deadlines. This position will track program outcomes and compile data and special reports summarizing the program's performance.

Essential Functions and Responsibilities:

1. 
   

   Develop metrics to determine program success and develop ways to increase service recipient feedback.

   
   
2. 
   

   Assess the needs of the program as it relates to data collection and efficiency.

   
   
3. 
   

   Track data to assess timeliness of completion and team performance outcomes.

   
   
4. 
   

   Create forms and PDF fillables to streamline work production and increase efficiency.

   
   
5. 
   

   Develop forms and templates to track the collection of data related to program outcomes and performance.

   
   
6. 
   

   Identify gaps in programming and collaborate with program administration to develop solutions to improve the efficiency of work.

   
   
7. 
   

   Routinely audit the federal and internal database systems and routine reports to verify that documentation is being entered appropriately.

   
   
8. 
   

   Send weekly and daily reports related to compliance deadlines.

   
   
9. 
   

   Conduct internal audits of files to ensure all program files are in audit-ready condition.

   
   
10. 
    

    Complete a full audit for all closed cases within a week of their closeout date.

    
    
11. 
    

    Collaborate with team members to finalize documentation, close out files and prepare them for storage.

    
    
12. 
    

    Respond to file requests in a timely manner.

    
    
13. 
    

    Exit youth from the internal database and input release data for the purpose of tracking outcomes.

    
    
14. 
    

    Assist in the creation of monthly and quarterly reports.

    
    
15. 
    

    Assist with the collecting and filing of required documents, as needed.

    
    
16. 
    

    Collaborate with the technology department to modify the internal database to suit the needs of the program.

    
    
17. 
    

    Complete Mandated Reporter responsibilities in accordance with local state requirements when there is knowledge or suspicion that a child's safety may be at risk.

    
    
18. 
    

    Fulfill training responsibilities according to child welfare best practices, the Children's Home of Poughkeepsie and ORR regulations.

    
    
19. 
    

    Is familiar with and adheres to all agency policies and procedures as outlined in the UC Manual and Procedures and the agency Employee Handbook

    
    
20. 
    

    All other tasks as delegated by the Program Director or Assistant Executive Director.

MINIMUM Qualifications:

  High School Diploma or Equivalent

  Clerical experience

  Highly proficient in MS Excel/Google Sheets

  Bi-lingual Spanish- English

  Must complete and pass a pre-employment physical, PPD and drug screen

  
  

PREFERRED Qualifications:

  Associate's or Bachelor's degree

  Related experience with data collection, statistical analysis and streamlining processes
