SUMMARY

The Director of Social Services directs the overall social services operation in a larger facility according to federal, state, and local standards, guidelines, and regulations. Additionally, it is responsible for patient assessments, care planning, and helping patients and families adjust to their surroundings.

ESSENTIAL FUNCTIONS

The duties listed below are essential functions of the position that must be performed to effectively perform the position in at least an acceptable manner as defined by Front Porch. In the future, other duties may be assigned and considered essential functions of the position:

* Interview all new residents to assess their psychosocial needs. Secures information such as medical, psychological, and social factors contributing to the resident's situation.

* Counsels residents individually or with family regarding plans for meeting needs and aids residents in mobilizing inner capacities and environmental resources to improve social functioning.

* Refers residents to community health resources and other organizations.

* Educates patients and families regarding their rights and responsibilities, health care decision-making/advance directives, effective problem-solving, and available community resources.

* Provides therapeutic interventions to assist patients in coping with their transition and adjustment to a long-term care facility and to address behavior or mood problems

* Performs admission and discharge planning coordination.

* Prepares timely documentation of social service interventions and assessments in resident charts.

* Conducts family meetings or family council/support groups.

* Integrates social services with the other elements of the resident's care plan through staff meetings and/or individual conferences.

* Coordinates with the nursing department to meet the residents' optical, dental, and audiological needs and arrange transportation.

* Coordinate continuum of care within the Continuing Care Retirement Community (CCRC).

* Perform all other related duties as assigned.
