Location Site: Salem Hospital, 890 Oak Street SE, Salem, Oregon 97301 Your role with us As a care manager, you will screen patients on admission to identify needs, coordinate care, and manage discharges. Collaborate with the multidisciplinary team, providing emotional support, crisis intervention, and brief counseling. Address high-risk patient needs, facilitate communication, and ensure resource utilization. Support transitions, conduct psycho/social assessments, and lead complex care initiatives. Leveraging your medical expertise and customer-centric mindset to employ a variety of treatment modalities, assisting patients and families in alleviating or resolving social, financial, and emotional problems related to illness, health care, and rehabilitation.
