SALARY DESCRIPTION: $48,889.61 to $55,159.44

PRIMARY FUNCTION: Responsible for managing and supervising the implementation of the Targeted Case Management Program and its staff.

PRIMARY DUTIES:

  1. Provide support, monitor, audit, and evaluate TCM Program and/or staff and the day-to-day operations to include, but not limited to: develop/maintain a welcoming/friendly/recovery orientated environment, collaborate with applicable stakeholders (i.e.; BHD), provide coverage for staff as needed (may perform case management duties and/or transport clients), training (i.e.; on-the-job), mentoring, problem resolution, policies and procedures, locating community based resources, provide feedback, review documentation, etc. E
  2. Manage and promote all aspects of providing individual person centered, strength based, culturally intelligent, and trauma-informed focused support to consumers to include, but not limited to: transport consumers as needed. E
  3. Supervise, evaluate, manage, and train/develop staff in accordance with the policies and procedures established by the agency and work closely with the Department of Human Resources to manage all personnel items (i.e.; interviewing, hiring, discipline actions, etc.). E
  4. Ensure timely submission of progress notes by TCM staff, service authorization approval and submission by BHD, protective payee responsibilities, and weekly billing submission. E
  5. Follow legal, organizational, and contractual requirements, laws and policies. E
  6. Ensure compliance with legal, organizational, and contractual requirements, laws and policies to include, but not limited to: audits, program evaluations and recommendations, licensing standards, new or enhanced policies and procedures. E
  7. Prepare, complete, and submit required items by due dates and required timelines (i.e.; reports, statistical data, quality assurance surveys, etc.). E
  8. Establish and maintain good communication, collaboration, and cooperation with all stakeholders (i.e.; staff, consumers, other treatment providers, BHD, etc.) NE
  9. Implement and/or practice the duties and responsibilities of a mandated reporter of suspected child abuse under current policies and procedures, as required. E
  10. Attend all meetings and professional development activities, as directed. NE
  11. Perform other duties as assigned. NE

The employee is regularly required to drive, stand, sit, reach, stoop, bend, and walk. Employee is frequently required to see, talk, or hear. Finger dexterity is also required. Infrequent lifting, including files, will be encountered.

WORKING CONDITIONS:

Work is generally performed in field and office environments, including travel to client homes for appointments and /or court hearings. Local travel required; some state-wide travel as needed. Flexible hours required; as dictated by program needs.

REPORTS TO: Adult Services Coordinator

QUALIFICATIONS:

  1. Minimum of a Bachelor's Degree from an accredited school in Social Work or a related field required.
  2. Master's degree from an accredited school in Social Work or related field preferred.
  3. Minimum of five (5) years of experience working with individuals with significant mental health, substance abuse, or co-occurring needs required.
  4. Minimum of one (1) year of practical supervisory experience; including supervision of staff.
  5. Strong cultural competency and interpersonal relationship skills.
  6. Strong communication skills, written and verbal, appropriate to target audiences across multiple functional areas, categories, levels, linguistic, and economic backgrounds; both in a team and individual environment.
  7. Excellent critical thinking; able to use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.
  8. Highly developed organizational skills and able to manage multiple priorities.
  9. Knowledge and experience with experience in using Microsoft Office suite.
  10. Able to work flexible hours, including evenings and weekends depending on needs of the program needs.
  11. Must successfully complete and pass all background checks.
  12. Transportation, valid Wisconsin Driver's license, state minimum auto insurance, and must meet La Causa, Inc. driving standards.
