Spanish-speaking candidates are especially encouraged to apply and will have the opportunity to make an even greater impact. *** *What You'll Do:* As a *Patient Care Coordinator*, you'll facilitate a collaborative process that assesses, coordinates, and monitors patient needs-guiding them through every step of their healthcare journey. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
