Job Summary As a Community Health Worker, the PASO Resource Specialist plans, develops, markets, and delivers public health education to local health program participants at Prisma Health and community groups. Facilitates enrollment of eligible participants in Medicaid, WIC and SNAP Programs. The Resource Specialist position is based out of the PASOs Program office in the designated market. In addition to educating individuals/groups, the Resource Specialist supports the PASO Program Manager in outreach activities, recruitment and resource navigation.Accountabilities * The Resource Specialist will focus education and enrollment efforts in the designated area of South Carolina. 25% * Travels to areas where there are large Latino populations 6 times per year (every other month) to offer information and enrollment services. Collaborates with local PASO Coordinators to connect with community groups.10% * Educates 50 people per month, for a total of 600 individuals per year. Each educational session will be 30 minutes. All people who are educated will be offered insurance enrollment support and other services. 25% * Assists the PASO Program by organizing at least 1 community outreach event per month where both PASOs and ACA information is offered. The Resource Specialist will promote the PASO educational offerings in addition to insurance information. 20% * Offers assistance in helping individuals connect to needed resources to reach PASO target goals for the designated area. 10% * The Resource Specialist will support Inpatient/Outpatient Social Work and Case Management staff as they work with Spanish Speaking Hispanic/Latino patients .5% * Collects data to report on program outcomes.5% Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor, manager, director or executive.
