Responsibilities

The Discharge Planner is responsible for coordination of discharge planning services within the Social Services Department. These duties include but are not limited to daily audit to ensure effectiveness of clinical plans, verification of communication with appropriate supports and partners, direct discharge planning and case coordination assignments, provision of reports and development of discharge process improvement as directed by the Director of Clinical Services.

Qualifications

Job Requirements

  • MSW, CSW, and/or Master's Degree in a clinical field preferred. An equivalent combination of education and experience may substitute. Supervision of a registered nurse, social worker or other qualified person is required.

Job Duties/Responsibilities

  • Responsible for coordination of discharge planning services within the Utilization Review Department
  • Daily audit to ensure effectiveness of clinical plans
  • Verification of communication with appropriate supports and partners
  • Direct discharge planning and case coordination assignments
  • Provides reports and develops discharge process improvement as directed by the Director of Utilization Review
  • Ability to define problems, collect data, establish facts and draw valid conclusions.
  • Must be able to express or exchange ideas by means of spoken and written word.
