Montefiore Medical Center is seeking a skilled and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our multidisciplinary kidney transplant team. This outpatient-only role focuses on the comprehensive care and management of kidney transplant recipients in the post-transplant phase. The post-transplant coordinator plays a critical role in managing immunosuppressive therapy, detecting early signs of rejection or infection, and guiding patients through complex clinical and psychosocial needs. This provider will serve as a key clinical resource, ensuring transplant recipients maintain optimal graft function, medication adherence, and long-term wellness. Responsibilities:
