TRANSPLANT COORDINATOR PRACTITIONER

Montefiore Medical Center is seeking a skilled and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our multidisciplinary kidney transplant team. This outpatient-only role focuses on the comprehensive care and management of kidney transplant recipients in the post-transplant phase. The post-transplant coordinator plays a critical role in managing immunosuppressive therapy, detecting early signs of rejection or infection, and guiding patients through complex clinical and psychosocial needs. This provider will serve as a key clinical resource, ensuring transplant recipients maintain optimal graft function, medication adherence, and long-term wellness.

Responsibilities:



  • Serve as the primary clinical provider for post-kidney transplant patients in the outpatient setting.
  • Perform physical assessments, develop and manage individualized treatment plans, and monitor for graft function, infection, rejection, and other complications.
  • Manage and adjust immunosuppressive therapies in collaboration with transplant nephrologists.
  • Provide extensive patient and family education on post-transplant care, including medication regimens, lab monitoring, and lifestyle recommendations.
  • Coordinate care transitions, facilitate timely follow-up, and collaborate with multidisciplinary team members to ensure continuity and quality of care.
  • Participate in performance improvement and transplant program development initiatives.



Qualifications:

For Nurse Practitioners (NPs):



  • Current NYS licensure as a Registered Nurse and Nurse Practitioner.
  • Minimum 2 years of RN experience required.
  • New NP graduates are welcome to apply.


For Physician Assistants (PAs):



  • Graduation from an accredited PA program and current NYS licensure as a Physician Assistant.
  • Minimum 2 years of clinical experience in a complex care setting.


Preferred Qualifications:



  • Experience in transplant medicine, particularly kidney transplant, strongly preferred.
  • Spanish-speaking candidates are highly encouraged to apply.
  • Demonstrated ability to work independently and collaboratively in a fast-paced, interdisciplinary environment.
