Our clinic offers services in family medicine, obstetrics and gynecology, social services, patient education, and referrals to specialty care. We are a Certified Health Care Home and offer enhanced care coordination services. What you will do as a Community Health Worker: This Community Health Worker (CHW) role is for Smiley's Behavioral Health Home (BHH) program. The BHH program provides person-centered, multidisciplinarycare to adults and youth with mental health diagnoses. The program focuses on integrating mental health and primary care to help individuals achieve their health and wellness goals through coordinated services and support. The BHH CHW works closely with the BHH Social Worker and meets weekly with the Faculty Psychologist to coordinate care for approximately 50 patients. Provide Direct Services such as wellness, health information, social services, disease prevention, and social support. adherence to patients and providers May organize and/or facilitate support groups Bridge the gap between communities and the health and social service systems by enhancing care quality by aiding communication. Make referrals and coordinate services and enroll patients into programs such as health insurance and public assistance Partners with patient/care-giver/management in care/decision making. What you will need: Community Health Worker Certificate at time of hire or completed within 6 months of hire 3 years' experience working on community health projects, programs or initiatives.
