Overview

The Clinical Dietitian is responsible for the nutrition screening, assessment, monitoring, evaluation, and education of Cardiac rehab patients through an integrated approach to nutrition care.

Certifications: Driver's License - Secretary of State (SOS); Registered Dietitian (RD) - Commission on Dietetic Registration (CDR); Licensed Dietitian Nutritionist - Illinois Department of Financial and Professional Regulation (IDFPR), Education: Bachelor's Degree, Work Experience:
Responsibilities

PROVIDES COMPREHENSIVE NUTRITION CARE OF PATIENTS ACCORDING TO POLICIES & PROCEDURES

  • Responsible for nutritional assessment completions 100% per policy
  • Meets charting guidelines 100% of the time determined by chart audits
  • Provides quality medical nutrition therapy as appropriate on cardiac rehab patients including necessary follows and in accordance with facility policy


ACTIVELY PROVIDES NUTRITION SUPPORT THROUGH ASSESSMENT, DEVELOPMENT OF A NUTRITION CARE PLAN, CONDUCTION OF LIVE COOKING COURSES AND NUTRITION WORKSHOPS

  • Works with other Cardiac Rehab Team members to provide nutrition therapy to patients in a timely manner.
  • Writes and manages care plans for all patients to be kept in their files with the department
  • Performs 1:1 consultations with all newly enrolled cardiac rehab patients as appropriate
  • Conducts nutrition workshops to cardiac rehab classes as outlined in the education calendar
  • Performs live cooking courses for cardiac rehab patients on a weekly basis as outlined by the department


PROVIDES NUTRITION CARE OF PATIENTS ACCORDING TO REGULATORY BODIES SUCH AS JCAHO, IDPH, CARF, ETC. AND CURRENT TRENDS AND KNOWLEDGE ACQUIRED ANNUALLY

  • Maintains knowledge of regulations
  • Analyzes performance compared to regulations and offers ways to better, efficiently comply to provide optimum nutrition care
  • Fulfills CDR and IDPR education requirements and maintains report portfolio with education receipts for IDPH or CDR audit


PROVIDES NUTRITION EDUCATION TO PATIENTS, FAMILIES, COMMUNITY EVENTS, STAFF GROUPS, ETC.

  • Develops and utilizes a variety of processes to determine patient education needs, ie consults, assessments, etc.
  • Works cooperatively with cardiac rehab team members in determining and providing patients' education needs
  • Provides nutrition education to patients, family members, staff, and community groups


Must be able to travel to conduct consultations, workshops, and live cooking courses between all Cardiac Rehab regional sites

Compensation and Benefits

The compensation range for this position is $23.52per hour - $39.28per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
