FTE: .8, Shift: Days Schedule: Exempt Position Summary Through use of the nursing process, care coordination, and case management principles, this Case Manager's (CM) primary focus is on achieving optimal patient health care outcomes while ensuring appropriate utilization of health care resources for those patients. The CM coordinates the care of patients at high risk for complications of chronic illness or high cost utilization. Working closely with primary care teams, specialty care teams and medical providers, the CM establishes a collaborative plan of care to assure adherence to the medical plan, improvement in functional status, and improved ability to self manage. Interactions with patients are time limited and guided by entrance and exit criteria. The CM serves as the liaison across the internal MHS care continuum and between MHS and all externally contracted providers, facilities, and resources. Job responsibilities are primarily performed over the telephone.Requirements

As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $112,674.00 - $183,810.00 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education.
