Responsibilities

* Assess patients medical and behavioral health needs

* Review all patient medications and check for potential drug interactions

* Ensure that the patient all providers who prescribe medications for the patient have an accurate medication list and holistic treatment plans

* Refer patients to community resources

* Create an integrated health treatment plan with patient engagement

* Follow up with referral sources to ensure that patients and other providers are following their treatment plan

* Obtain health information from patients including but not limited to height, weight, waist circumference, allergies, current and previous medications, and smoking

* Responsible for ensuring that one's caseload is meeting health performance indicators

* Enter patient health information into computer system

* Assist patients in completing patient assistance and other public assistance forms

* Assist patients in completing Integrated Health Home enrollment forms

* Assist patients in completing health questioners

* Provide direct services to patients as needed including but not limited to transportation

* Assist patients in completing quality assurance surveys

* Day travel to satellite clinics may be required. Mileage for travel will be paid


Qualifications

REQUIRED QUALIFICATIONS:

* License RN or higher

* Six month's experience working in a medical clinic, home health, hospital, or nursing home as an employee or intern.

* A valid driver's license, auto insurance and reliable transportation

* Computer Skills

PREFERRED QUALIFICATIONS:

* Experience providing behavioral health services
