Summary

The lead advanced practice provider (APP) at the Ponce de Leon Center is a member of the clinical team that serves as the primary care provider for people living w/ or affected by HIV who are enrolled at Ponce. The Lead APP assumes direct patient care responsibilities, which may include maintaining a continuity patient panel, performing enrollment visits for new patients/patients re-entering care, staffing the Walk In/Acute Care Center to provide same day care to Ponce patients with acute/urgent needs, performing Pre-exposure prophylaxis (PrEP) visits within the Ponce PrEP program. In addition to direct patient care, the lead APP assists with administrative and operational duties with an emphasis on monitoring and evaluating clinical quality, through the oversight of onboarding/training of new APPs/clinical team members, leading quality improvement activities, and creating ongoing educational opportunities for the Ponce clinical team.

Qualifications:

Required:

* Master's Degree in Nursing as Advanced Practice Nurse

* Master's Degree as a Physician Assistant from an accredited PA program

* Minimum 2 years experience as Ponce APP or equivalent experience in external organization providing care to people with and affected by HIV

* Current Georgia license as a Registered Professional Nurse and current Georgia Authorization/Certification as a NP/ Advanced Practice Registered Professional Nurse and prescriptive authority through Georgia State Composite Medical Board is required for NPs.

* Current Georgia license as a Physician Assistant required for PAs

* Basic Life Support (BLS) certification required

Preferred:

* Leadership experience or training in a clinical setting

* Training and/or experience in quality improvement, program evaluation or data management

* Formal leadership training or certification

* Quality improvement training or certification (ie. CPQH, Six Sigma, etc
