Loudoun County Department of Family Services seeks a dedicated and experienced professional for an Adult Protective Services Ongoing Specialist position. The successful candidate will join the Adult and Aging Unit within the Department of Family Services which envisions better health, safety, and well-being for all. Job Summary

This APS Ongoing Specialist position provides critical case management services to older adults and adults with disabilities, in cases where adult abuse, neglect or exploitation has been substantiated.

  • Providing intensive case management services to stop or prevent further abuse, neglect or exploitation of older adults or adults with disabilities after a substantiated APS investigation.
  • Respond to emergency calls and provide immediate assistance to vulnerable adults.
  • Respond to requests for assistance with repatriation cases including coordinating with outside agencies, companion assessments, and providing support to the repatriated adult upon arrival.
  • Advocate for clients' rights and connect them with appropriate resources and services.
  • Developing and implementing individualized service plans tailored to the specific needs of each client.
  • Coordinating and monitoring services such as medical care, benefits services, and housing support.
  • Conducting Long Term Supports and Services screenings to determine eligibility and appropriate interventions.
  • Performing Assisted Living Facility screenings to assess the suitability of residential placements.
  • Collaborating with healthcare providers, social workers, and other stakeholders to ensure comprehensive care.
  • Maintain accurate and detailed case records, ensuring compliance with all regulatory requirements.
  • Interpret laws, policies, and regulations related to Adult Services and Adult Protective Services programs.
  • Conduct home visits to assess clients' needs and monitor service delivery.
  • Inform clients of related services programs, rules, regulations, and their rights to participate.
  • Ensure accurate and timely data entry into required systems.
  • Complete necessary federal, state, and local planning and reporting requirements.
  • Coordinate with other agencies and participate in multi-disciplinary and inter-agency teams to provide comprehensive services to clients.
  • Conduct community presentations and serve as a resource to clients and the community in areas of expertise.
  • May perform other essential job functions specific to the position and department assignment.

Salary commensurate with experience.

Minimum Qualifications

Bachelor's degree in a Human Services field; or a Bachelor's degree in any field with a minimum of two (2) years of directly related experience in a human services area.

Preferred Qualifications:

  • Comprehensive knowledge of Adult Protective Services/Adult Services/Case management programs and procedures.
  • Significant experience in conducting APS investigations and providing APS on-going case management services to older adults and/or adults with disabilities.
  • Knowledge of social work theory, casework methods, and community organization/resources.
  • Understanding of human behavior and the behavioral sciences, particularly concerning human development, aging, family dynamics, and interpersonal relationships.
  • Ability to engage in sensitive discussions with clients about various aspects of their lives.
  • Capacity to respond effectively in crisis situations.
  • Master's degree in Social Work, Gerontology, or a related field.
  • Experience working in local government or a department of social services.
  • Bi-lingual skills in Spanish and English.

Job Contingencies and Special Requirements

Pass pre-employment background checks to include criminal, credit, CPS/APS, and DMV. Valid driver's license and good driving record (driving records may be reviewed annually for continued qualification). Hold or obtain certifications such as CPR if required for assigned position. Obtain specific training such as CPS/Foster Care/APS state training as required or desired for assigned area.
