Job Summary

The Transfer Center Coordinator plays a critical role in supporting the efficient and timely coordination of patient transfers into and out of the hospital. Working under the direction of Transfer Center Nurses, this position assists with arranging non-medical transportation, uploading and managing clinical documentation, verifying insurance for in-network coverage, and completing a range of administrative tasks to facilitate safe and seamless patient transitions.

Essential Functions:

  • Coordinate and arrange non-medical transportation services (e.g., taxi vouchers, Medicaid cabs, wheelchair transport) for patients during transfers or discharges. (25%)
  • Collect, organize, and upload clinical documentation to support transfer requests, provider consultations, and admissions. (25%)
  • Assist in the assessment of insurance status and verify in-network coverage to support financial clearance and care coordination. (15%)
  • Communicate professionally with internal departments, external providers, transport vendors, and insurance payers. (15%)
  • Maintain accurate documentation in the electronic health record (EHR) and transfer tracking systems. (10%)
  • Support Transfer Center Nurses in administrative functions and act as a liaison for operational follow-up. (5%)
  • Perform other duties as assigned to support overall transfer center operations and patient flow. (5%)

Education:

  • High School Diploma or GED Required
  • Associate's Degree Preferred

Work Experience:

  • 1-3 years At least 3 years of experience in a healthcare setting, with preference for roles involving coordination, patient access, or clinical support. Required and
  • 1-3 years Experience in a hospital transfer center, call center, or transport coordination

Licenses:

  • CMA-Certified Medical Assistant - AAMA - American Association of Medical Assistants Required or
  • EMTB-EMT Basic - CO - State of Colorado Required or
  • CNA - Certified Nursing Assistant - DORA - Department of Regulatory Agencies Required

Knowledge, Skills and Abilities:

  • Completion of a Medical Assistant (MA), Certified Nursing Assistant (CNA), or Emergency Medical Technician (EMT) program required.
  • Working knowledge of HIPAA, medical terminology, and clinical workflows.
  • Strong computer skills, including experience with electronic health records and Microsoft Office tools.
  • Effective communication, organization, and multitasking skills.
  • Familiarity with insurance verification, authorizations, and provider network concepts.

$20.99 - $29.39 / hr
