Position Title: Patient Care Advocate

Work Location: Pennsylvania, Eastern Standard Time, Reside in PA, Travel may be asked but may be limited and infrequent

Assignment Duration: Potential to Extend

Work Schedule: 8:00 a.m. - 5:00 p.m., 8:30 a.m. - 5:30 p.m. Eastern

Work Arrangement: Fully remote, travel 25% - 50% (at most)

Position Summary: Telephonic outreach to members, addressing gaps in care, barriers to care, and improving member experience.

Background & Context:



  • This role is a new role focused on Quality Medicare Hiring to increase member retention and experience.

    Key Responsibilities:
  • Telephonic outreach to members, addressing gaps in care, barriers to care i.e., transportation, offering in-home test kits for gap closures.
  • Addressing member concerns and identify solutions to aid in the improvement of the member experience.
  • Collaborate with other team to solve member concerns.
  • Outreaching members for gaps in care and/or additional training.

    Qualification & Experience:
  • RN must have active license.
  • Customer service skills and Motivational interviewing.
  • Ability to multi-task.
  • Technology skills.
  • Bachelor's Degree in Healthcare, Public Health, Registered Nurse, Psychology, Social Work, Health Administration, or related health field or equivalent work experience.
  • Preferred: Associate Nursing Degree, Project Management/Lead, Case Management.
