Access to Care helps people who don't qualify for public or private healthcare coverage programs get the healthcare they need. CarePartners / CarePartners Plus Programs This role carries a caseload, working with low income individuals, helping people that do not qualify for public or private healthcare coverage programs get the coverage they need; as well as, helping qualifying members with choosing an insurance provider and plan that fits their needs from the healthcare marketplace. **This position is currently located in Portland, ME. This office is moving to South Portland effective September 2025** Summary: The Case Manager - Medical Outreach role is responsible for the provision of case management services and achievement of positive intervention outcomes for assigned clients. They work on-going with patient caseloads, delving deeper into more medically and socially challenging issues and health needs, addressing barriers for patients.Required Minimum Knowledge, Skills, and Abilities (KSAs)
