Description MH Corporate Professional - Nonclinical Req #: 39694 CHI Access to Care Coverage, Case Manager - Medical Outreach Full Time, 40 hrs per week, Day Shift Since 2001, our nationally-recognized programs have helped hundreds of thousands of community members access: Affordable health coverage and high-quality health care Free and low-cost prescription medications Assistance with transportation, housing, food and health care system navigation Access to Care helps people who don't qualify for public or private healthcare coverage programs get the healthcare they need. CarePartners / CarePartners Plus Programs This role carries a caseload, working with low income individuals, helping people that do not qualify for public or private healthcare coverage programs get the coverage they need; as well as, helping qualifying members with choosing an insurance provider and plan that fits their needs from the healthcare marketplace. This position is currently located in Portland, ME. This office is moving to South Portland effective September 2025 Summary: The Case Manager - Medical Outreach role is responsible for the provision of case management services and achievement of positive intervention outcomes for assigned clients. They work on-going with patient caseloads, delving deeper into more medically and socially challenging issues and health needs, addressing barriers for patients. Required Minimum Knowledge, Skills, and Abilities (KSAs) Education: Bachelor's Degree preferred, with a specialization in Psychology, Behavioral Health, Social Work, Special Education, Counseling, Rehabilitation, Nursing, or closely related field preferred. License/Certifications: Current applicable state(s) license as a Licensed Social Worker preferred. Valid driver's license and reliable vehicle required. Experience: With a bachelor's degree: Two years of experience in a community health setting, including one year of case management experience required. In lieu of bachelor's degree: Three years of directly relevant experience in a healthcare setting, including 6 months of case management experience. Experience collaborating with state and federal agencies, community agencies, municipal, and public programs preferred.
