Safe Haven offers a set of supportive services for individuals who are unhoused and actively using substances, often with co-occurring mental health challenges.

The Safe Haven team is a multidisciplinary team offering psychiatry, nursing, case management, peer recovery support, hospital liaison, and housing support.

The primary focus for this position is care coordination, which addresses the social determinants of health to improve health and well-being. The Case Manager/and Housing Specialist are integral to this goal, working with clients to remove barriers to care. The Support includes:


  • Connection to emergency shelter and housing, access to basic needs,
  • Primary care coordination,
  • Access to benefits/entitlements, navigation of legal issues,
  • Building improved natural supports and social environment.

Education/Licensing Requirements:



  • Associate Degree in social Services or Bachelor's degree in social work, psychology, or other related human services field, with at least one year of relevant experience.

  • Must have a valid driver's license, reliable transportation, and vehicle insurance.
  • Bilingual applicants are strongly encouraged to apply!

Duties and Responsibilities :


  • Conducts comprehensive housing assessments and assists clients to find and maintain safe, affordable housing.

  • Develops relationships with landlords to expand the array of housing options available to our clients.
  • Participates in case reviews.
  • Is familiar with the Coordinated Entry System and the resources available to individuals who are unhoused or unstably housed.
  • Requires excellent verbal and written communication skills.
  • Demonstrates an understanding of mental health and substance use issues.
  • Can develop treatment plans and use an EHR.

  • Connection to basic needs, including but not limited to food, clothing, utility or rental assistance, etc.

  • Primary care coordination - help with scheduling and maintaining appointments, as well as adhering to medical recommendations.

  • Access to benefits/entitlements, such as GPA, SSI/SSDI.


  • Navigation of legal issues,

  • Building improved natural supports and social environment.

  • Connect with education/employment resources/opportunities.
