Pay information



  • Pay Level: 024
  • FLSA Category: Exempt - Salaried
  • Individual pay will be based on skills, experience, qualifications, location, internal equity, and other relevant factors.

A day in the life

The Team Leader of Population Health is responsible for leading and providing day-to-day management support to a team of case managers and non-clinical staff that provide assessment, care planning and coordination, and advocacy services to an assigned patient base. This role directly supervises staff, coordinates workload, delegates responsibilities and provides guidance in developing and communicating integrated care plans, ensuring ongoing communication and collaboration between internal and external stakeholders.

Your responsibilities will include



  • Directly supervises and guides employees in accomplishing work objectives within the unit. Coaches, motivates, and recognizes employees. Sets clear goals and objectives and performance expectations and holds employees accountable for achieving results.
  • Works collaboratively with management to ensure program goals, projects and initiatives are implemented and meet department workflows, standards, and policies.
  • Monitors daily case management workload and delegates responsibilities to the team. Handles escalation of critical items or issues and helps the team determine a forward path for resolution.
  • Ensures continuity of care with external managed care organizations and arranges referrals for ongoing care, as needed.
  • Assists in the development and administration of case management policies and procedures with the goal of balancing quality patient outcomes and cost-effectiveness.
  • Measures effectiveness of case management, quality, or utilization management initiatives by performing individual case management documentation audits, tracking performance metrics, and delivering feedback.
  • Establishes and maintains business relationships with key internal and external stakeholders.
  • Reviews team activities and operations for compliance with established policies and standards.
  • Initiates new process development and supports improvement efforts to ensure the department is operating to the highest of quality and efficiency.
  • Oversees the clinical training program for the team and addresses individual and departmental training needs.
  • Participates in special projects, cross-functional workgroups, or corporate projects, as assigned.



What you need to succeed



  • Associate's degree in Nursing
  • 5 years of clinical nursing experience, of which at least 2 years in case management, disease management, health coaching or related experience.
  • 1 year or more of leadership is a plus!
  • Be a problem solver who researches, analyzes and defines methods, practices, and procedures to resolve issues.
  • Clearly communicate and support employees growth and performance, which supports our mission of improving the healthcare and well-being of our members.
  • Bring a positive, collaborative mindset to the team to continue to build a great culture.



Equivalent combination of education, experience or training determined to be acceptable by Human Resources may be substituted, unless regulated by contract or program standards
