Job Description/Essential Functions: |
Horizon House Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities and homelessness to help them stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice. We are seeking qualified candidates interested in joining our Residential Services Group Home team at our Wilmington, Delaware location.
Work Schedule Full Time 40 hours per week, Mon-Fri, 3P-11P
The Certified Nursing Assistant would provide direct care and supervision to participants in the residential group home setting and ensure a safe, secure, healthy, and recovery-oriented environment which promotes growth and development. They will also facilitate participant growth and development towards independence in the area of self- care, daily living, socialization and community integration. Some of the responsibilities would include the following:

Provide guidance and support to participants, with the development of a cooperative and trusting relationship. -
Reviews individual recovery plans regularly to ensure knowledge of the rehabilitation process. -
Administers treatments by assisting with medications and training on self -medication practices. -
Assess participant's conditions by monitoring vital signs, recording intake and output and documenting participant care. -
Report observations about participant's skills and health issues to the team effectively and clearly. -
Trains participants in the completion of instruments of activities of daily living (household management).

Job Qualifications: |

High school diploma/GED and current Certified Nursing Assistant certification in the state of Delaware. -
Knowledge and ability to provide hands on assistance with activities of daily living and instrumental activities of daily living. -
Good communication skills both oral and written. -
Knowledge and ability to develop effective rehabilitative relationships with participants receiving supports and services. -
Experience in mental health services or comparable direct service. -
Ability to respond to disruptive behavior, agitation and depression. -
Evidence of attitude to work in an environment that requires patience, flexibility and initiative. -
Possess a valid driver's license with an acceptable driving record.
Scope:

Requires the ability to lift, carry, fold, kneel, reach, stack, stoop, and bend which includes direct interventions with participants. -
Ability to climb and descend stairs to a second or basement level. -
Possess manual dexterity and fine motor skills. -
Requires crisis intervention techniques in crisis situations.
