Functions independently and/or in conjunction with an interdisciplinary team based on the patient's identified needs. Practice is within the scope defined by the Washington State Nurse Practice Act, based on the nursing process model and concept of community health nursing. About the Team Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
