Our Safe Haven Integrated Health Home team provides supportive services to individuals challenged by homelessness, substance use and mental health issues. We use a harm reduction, person-centered approach. You will work with a team of professionals - a clinician, prescriber, nurse, and case manager, all of whom work in concert to provide the highest quality care. You will provide clinical and support services for individuals and their families who are coping with long-term needs associated with homelessness, mental illness and substance use. Develop, design and implement person-centered recovery plans to help people maintain their health and well-being. We need someone who can demonstrate strong writing, communication, computer skills, and documentation skills. You must have the ability to multitask, be organized, and have excellent time management skills. Transportation of clients in your vehicle to medical and community appointments is required.
