The position involves support and evaluation of the patient and their experience following discharge. Summary of responsibilities include: Ability to effectively triage clinical issues by using an approved, evidence based resource. Review discharge instructions and ensure understanding. Determine if there are questions or concerns following the hospital stay. Escalate issues or concerns by notifying appropriate parties (e.g. Case Management, Physician, Clinical Director, Patient Relations, etc.) Summarizing responses in a database for documentation, trend analysis and follow-up. Will have a formal dotted line reporting relationship to the CNO if the Director of Patient Family Centered Care does not have an RN. Experience Required Minimum of 10 years of direct patient care on a patient care unit in an acute care hospital Education Requirements Graduate of NLN accredited School of Nursing. License/Certification Requirements Current NJ-RN License (Registered Nurse). Special Requirements Scheduled Days / Hours: Monday- Friday; License (or Certification):NJ-RN Required Effective problem-solving skills, including the ability to analyze complex situations, draw conclusions and implement appropriate follow-up actions appropriately and efficiently. Experience with windows-based computer programs and ability to use computer for data analysis and data display required. Excellent interpersonal skills; attention to detail; willingness to learn new tasks; commitment to Patient -Family Centered Care; ability to work independently. |
