Overview Supporting Community Healthcare is a rewarding role. LifeLong Medical Care is looking for a Community Health Worker (CHW) - Prenatal to work at our William Jenkins Health Center in Richmond, CA. The Prenatal CHW's responsibilities include welcoming our patients, making basic psychosocial and nutritional assessments, offering appropriate information, education and referrals, and providing emotional support to a culturally and socioeconomically diverse population. The CHW will work closely with the medical providers and medical assistants. The ideal candidate is someone who likes to work with a team, is familiar with local community resources, and has experience working with women or in a community health setting. This is a full time, 40 hrs/wk, benefit eligible position. This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA. LifeLong Medical Care is a large, multi-site, Federally Qualified Health Center (FQHC) with a rich history of providing innovative healthcare and social services to a wonderfully diverse patient community. Provide orientation and complete assessments on pregnant patients
