Community Health Worker (CHW) Intern

This position will be for 120 total hours and pay $12.50 per hour plus mileage for travel(s).

Example Duties and Activities

• Assists with building relationships with NFP staff, NFP families, and networks with key community partners including Goodwill staff (community agencies, educational institutions, financial liaisons, and employers) to enhance the menu of wrap-around services provided

• Assists with creating easily accessible rapid-response and long-term solutions for NFP nurses and families to connect effectively to quality community resource networks

• Assists with ensuring timely and accurate entry of required information into NFP databases and files.

• Participates as needed in NFP CHW team meetings

• Participates in continuous innovation projects that improve operational efficiency and evidence-based outcomes

Required Competencies

• Completion (or pending certification) of Certified Community Health Worker (CCHW)

Other Requirements

• Has the ability to occasionally work a flexible schedule beyond normal business hours.

• Is willing to travel to participants' homes, workplaces, community centers, and other venues at times that are convenient to individuals being served.

• Background Screening - All candidates must pass a pre-employment background check. Must have a valid driver's license, up-to-date auto insurance, and a good driving record.
